Document artifact facade cards and context backfill (CF-A) 96c8404c5e 
Why: D46/D47 add inbox facade cards for truncated tool results and
artifact text backfill; guide needs the unified presentation story.
Evidence: usage.md 4.5 gains facade/backfill bullet.
Impact: docs-only.
