weichenandCursor 6fa933ac27 fix(kb): return offline client instead of Fatalf under require-online 
Library must not abort the process; E2E callers use EnsureOnline/t.Fatal
so ambient RUN_PI_E2E no longer kills short server tests.

weichenandCursor f3ed57c41f fix(kb): redact DSN logs and fail closed when KB must be online 
Add Online(), honor KB_REQUIRE_ONLINE/RUN_PI_E2E unless KB_ALLOW_OFFLINE,
and refuse empty/wrong credentials without leaking secrets in logs.
