Persist ComboNeedsConfirm plans at confirm tier without executing writes, then run task.run via OwnerMux and a stable idempotency key.

Add the app/intentflow skeleton so HITL state, error codes, REST shapes, and BatchActionPolicy live outside intent and default to the legacy tier.
