weichenandCursor 9daf3b7a10 fix(web): replay task phases after refresh and keep interrupted resumable 
Progress lived only in the open SSE stream, and interrupted history was shown as completed so the card could not be continued.

Hide submit/review/approve on none docs, surface the blocked-run hint in
chat, and warn that unmanaged duties cannot be added after activation.

Use deliverablesRef + limited poll for post-task server docs; pass
reconnectRunId across auto-retry; GetRun reads idempotency_key;
StartClaimedRun fails closed on UpdateRun; MessageBubble/E2E scope
