feat(tooling): add governance catalog and requirement overlap gates

Extend agentctl SQLite with catalog tables, CJK FTS5 recall, hard/soft
overlap gates, fail-open embeddings, backfill, and MCP/skills hooks so
cross-conversation designs cannot silently conflict.

Load configs/agent-governance.json from the main worktree with fail-closed
POLICY_* codes, and keep test fixtures writing the same contract file.

fix(ops): copy agentctl_w4 into script test fixtures
...
Disposable Git fixtures only copied agentctl.py, so imports of the new
W4 helper module crashed before any guard assertions could run.
