weichenandCursor f168e44ce7 feat(projectinstance): allow live workflow config edits except switching 
Activated projects can update duties, approval policy, and document enablement. Changing the workflow definition stays rejected, and project 143 remains frozen.

weichenandCursor f50692be25 fix(projectinstance): distinguish never-activated vs closed governing instances 
GetLiveInstance remains active-only; access ABAC now uses
GetGoverningInstanceForAccess so completed/terminated PG rows are not
treated as unrestricted legacy projects, while brand-new projects without
instances stay open for creators.
