任务作用域键归一化与失败对话落库

目标

新工作流实例使用 bd_* 任务键时，前端仍发送旧键（如 intake）不再返回 TASK_SCOPE_UNRESOLVED；若作用域仍无法解析，本轮用户消息与失败回复先落库。

POST /api/v1/tasks/runs 在 409 早退前写入 user + assistant 消息
