docs(task): record review repair run and evidence

Why: independent Review blocked the adoption; the repair unit ran through the shared executor and was integrated with agent-exec apply.
Evidence: .task/evidence/governance-adoption/ (SELF): governance check and agent-exec validate on the repaired head.
Impact: task card and evidence logs only.
