Record the two-level rehearsal and freeze verification

Why: Phase 3 requires evidence that real children flowed child -> initiative and that a frozen initiative rejects new children.
Evidence: make evidence SELF receipt for task-triage (NEW) and forced JOIN refusal (exit 7); PR #595/#596 Review and CI records.
