Why: Continue the existing delivery after explicit approval for one extra sample.

Evidence: Startup precheck READY and live output/run-limit fixture checks are recorded by evidence.

Impact: Fixed evaluation candidate, max two paid runs and no automatic retry; preserve all original failed samples.
