Record review-fix evidence on the chat-process-split-r1 task card

Why: the card and PR description are one document; the 2026-09-30 review
fixes (symlink untrack, per-turn live interleaving, artifact process mark)
need to appear on it.

Evidence: card lists frontend 22a8723/754ce83, backend ae8e5d0, Dev
596d02c with the verification outputs (vitest 27/27, go test ./... ok,
make test exit 0).
