Pin semantic review fixes for U3 r0.3

Why: Independent review found three unsupported-claim paths in the prior candidate.

Evidence: Each source-context, scope and unstructured-placement counterexample failed before repair; Backend full Go and selected fake-stream tests passed after.

Impact: Advance Backend 9019f90 and preserve the failed review plus exact prior combination receipt; revalidation follows on the new SHA.
