weichen 3d2da4eb0b Document card streaming three-tool flow in usage guide 
Why: CF-S (D44/D45) adds card_begin/card_append/card_commit; users need
the streaming flow and crash-fallback behavior documented.
Evidence: usage.md cardflow bullet updated to v2 flow.
Impact: docs-only.
