+## 用户目标与授权
+
+用户只上传文档、不输入文字时，界面显示 `请求失败（HTTP_400）`，助手没有任何回复。授权按已确认方案修复：turn 协议携带附件引用；无文字时做结构化梳理或解析引导，并给出下一步选项；有文字但未要求处理文档时，先回答原文再追问是否总结或更新。

+前端允许仅附件发送。`POST /api/v1/chat/stream` 在 `message == ""` 时直接 400，且前端不读响应体，信封里的 `message cannot be empty` 被丢弃。附件从未进入 turn。

+
+- 空文字且带附件时合成落库句「已上传文档：《文件名》」，并把附件引用放进当轮上下文。
+- 无文字：解析未完成则说明状态并询问；正文可读则梳理、审阅并挂 1/2/3 选项（总结、说明如何修订、先不处理）。
+- 有文字但意图没有消费文档：正常回复后追加一次追问。
+- 聊天 SSE 的 4xx 读取信封；`VALIDATION` 展示具体原因，而不是 `HTTP_400`。

+
+只上传文档、不输入文字时，`POST /api/v1/chat/stream` 返回 400 `message cannot be empty`。前端丢弃响应体，气泡显示 `请求失败（HTTP_400）`。

+## 二、 测试目标 (Test Objectives)
+
+- 无文字且无附件仍拒绝。
+- 无文字但有附件不再因空消息被拒绝；解析未完成时返回可阅读的引导，而不是 400。
+- 正文可读且没有对话模型时，回退为包含正文要点和下一步编号的回复。
+- 有文字且未要求处理附件时，在原回复后追问一次。
+- 4xx 信封能还原成具体文案。

+## 三、 测试方案与环境 (Test Plan & Environment)
+
+- OS：Linux；工作区 `.worktrees/attachment-only-turn-2026-09-28`；基线 `3e534bbb39d0d957e0b720a4ca06f22dd15959e6`。
+- 层级：工程单测与内存 HTTP。未重启 :8080 上已有的 `agent-server`（pid 801797），因此没有对正在运行的 dev 制品做浏览器复验。
+- 无对话后端时，梳理走模板回退，不把模板输出记成产品 PI PASS。

+## 四、 预期结果 (Expected Results)
+
+- 双空请求 400，正文含 `message cannot be empty`。
+- 仅附件且 `parse_state=pending` 的同步对话返回 200，回复含「解析」。
+- 可读正文的协调结果含正文片段和 `1.` 选项，并挂 3 个期望选项。
+- 无关文字追加「另外已收到」；总结意图不追加。
+- 前端把 `VALIDATION` + `message cannot be empty` 显示为「请输入文字，或上传文档后再发送」。
