W4 U12 workflow/run-control 独立 flag 测试报告

U12 不得与 U10/U11 共用开关。W4 只做独立 flag dry-run 证据。
二、 测试目标 (Test Objectives)
- 仅 U12 on 时注册 run-control adapter
workflow.run无 revision 可 dry-run；Denied fail-closed- 不升 PLAN_TIER

feature/w4-u9-segment-live；Go 单测 - Playwright：NOT RUN（不升 confirm）
