Why: The approved plan remained an earlier review snapshot and contradicted the index and task card after U1 implementation began.

Evidence: The pre-decision r0.1 file hash is f99e1424; the additive dated note preserves that text and records owner stage 6/7 decisions, four package reviews and remaining gates.

Impact: Documentation status only; no asset or product behavior change.
